Parent/caregiver supervision is required at all times. Little Elephants Discovery Studio staff may facilitate and guide activities, but are not responsible for the direct supervision or physical safety of your child. You must remain with your child throughout your visit. By completing this waiver, you acknowledge full parental/caregiver responsibility for your child's safety and well-being during your time in the studio.

Liability Waiver & Release Agreement

1. Assumption of Risk. I understand that participation in play activities at Little Elephants Discovery Studio ("Studio") involves certain inherent risks, including but not limited to falls, contact with other children, and injuries arising from the use of play equipment, sensory materials, and other studio features. I voluntarily accept and assume all such risks on behalf of my child.

2. Parental / Caregiver Supervision. I acknowledge and agree that I, as the parent or legal guardian, am solely responsible for the direct supervision and safety of my child at all times while on Studio premises. Studio staff and facilitators are present to guide and enrich activities, but are not designated supervisors or responsible for my child's physical safety. I will not leave my child unsupervised at any time during our visit.

3. Release of Liability. In consideration of my child's participation, I hereby release, waive, discharge, and covenant not to sue Little Elephants Discovery Studio LLC, ElevateChild Childcare & Development Center, their owners, operators, staff, agents, and employees from any and all claims, demands, losses, or causes of action arising out of or in connection with my child's participation in any Studio activities, whether caused by the negligence of Studio staff or otherwise, to the fullest extent permitted by applicable law.

4. Photo & Media Release. I grant permission for the Studio to photograph or record my child during Studio activities for marketing and promotional purposes, including social media. I may opt out below.

5. Health Representations. I confirm that my child is in suitable health to participate in play activities and that I have disclosed all known allergies, medical conditions, and special considerations in this registration form.

6. Rules & Policies. I agree to abide by all Studio rules, including the shoe-free policy, caregiver supervision requirement, and any instructions provided by Studio staff. I understand that failure to comply may result in removal from the Studio without refund.

7. Governing Law. This agreement shall be governed by the laws of the State of Arizona. If any provision is found unenforceable, the remaining provisions shall remain in full force.

Acknowledgements — all required

I understand and accept that I am fully responsible for supervising my child at all times during our visit. Studio staff are facilitators, not supervisors.

I have read and agree to the Liability Waiver & Release Agreement above, and I release Little Elephants Discovery Studio and ElevateChild from any claims arising from my child's participation.

I confirm that all information provided in this form is accurate and complete, and I have disclosed all known allergies or medical conditions relevant to my child's safety.

I grant permission for the Studio to use photos/videos of my child for marketing purposes. (Uncheck to opt out.)
